I have been on PPIs for the past 4.5 years. Specifically, I take aciphex, but all of the drugs in that class (nexium, prilosec, prevacid, and I'm sure several others) are pretty much the same. Although any drug, if taken long-term, has its fair share of risk, I don't believe the risk of complications from taking PPIs is very high. At least I hope not. I can't do without a PPI; I have an esophagael dysmotility disorder that cannot be fixed (I don't think) and have been told I will be on PPIs for life. On a positive note, PPIs have greatly increased my quality of life. Before aciphex, I could hardly eat.
