Posting for my brother who has Avascular Necrosis, Fibromialga, and RA...
Dx'd 2012
Methotrexate for the past year, up to 20mg (smaller doses were ineffective/wore off)
Liver enzymes last week were too high, so doc says he had to stop the MTX.

What med did any of you try next?
Is there a pyramid of drugs like with crohn's where they'll try things in a certain order with biologics last?
If his liver reacts this way to MTX, is there a "usual" for any other drugs that might work/not work?

I've been out of the crohnie circle for almost 4 years, so lots of these meds are new to me, and we're floundering for a place to start for him.

There are all the biological meds but whether they will prescribe them with high liver enzymes I don't know. There are others too, quite an arsenal out there but they may wait for the liver to settle before starting something new. My liver was upset at one stage but after it settled I as put on arava initially and then back on mtx but at a much smaller dose and sulphursolasine as well. Afterthat I had no white blood cells left so now I am on Humira and mtx.
Unfortunately it is a bit of a round about until they get the right cocktail of meds to suit you personally.

Golitho said it very good, there are many medications out there. Sulfur is an older medication and it's an antibotic type therapy as some people do get help with antibotic therapy...Have your brother google RA and antibotic therapies...that might be worth a shot, if it can help...
