Hello I just received I letter today in the mail from DAS Disability Adjudication Services. I have to fill out this form about my activites of a daily living. Heaven anyone gotten this letter yet?

I'm sorry I can't be of much helps since I've never had any dealings with Disability Adjudication Services. I'm sure there are other members who are familiar with it who will post later on.
