Sounds to me like your doctor is very understanding, and as long as you don't JUST use the pain meds for pain control and are trying other alternatives for your pain, then I don't see why your doctor would say no. If your doctor offers somethig else to try first, then take it, try it, then report back if it worked or not. But also doctors usually know that people become tolerent to pain meds and that it needs to be raised or some other medication tried, they expect it, it's part of being on this kind of medication.
If your doctor says no, then ask if he can refer you to a doctor who can treat your pain more better. Sometimes family doctors are only allowed to prescribe so much pain meds, I know if some states, it's the law that after a certain amout of naroctics mgs, they have to have a certified pain doctor prescribe stronger pain meds (I know in some states, it's like anything higher equivalent to 60mg in morphine daily), so don't get upset if they can't help you, as sometimes it's just the law, and sometimes doctors who are not experienced in narcotic pain meds want to refer people out because they don't want to endanger your life, so they are doing it for your benifet, not cause they think your a "drug seeker". Alot of family doctors are only experienced at prescribing them for short term, or at low doses for chronic pain.....
So just tell your doctor that the medication isnt helping like it was, tell them the truth. But don't throw a fit if he says no, cause then you will look like a seeker, ask for what they CAN do to help you. Living a life with chonic pain is a hard one, and it is hard when we don't want to be confused with the drug seekers, but it sounds to me like your doctor does understand, and the fact that he's already giving you pain meds means that he already trusts you, if he didn't then he wouldn't be giving you anything, so calm down and relax, things will go good.
