Oh Mindy,
If you haven't already, take the time to write down a LIST of surgeries and dates, MED you are taking and doses and who gave them to you, any OTC meds/suplements, and allergies and what the med does to you. Be very limited in what your write...sort of like section it off with titles: Current Medications: Xanax CR - 3mg- Dr. Jones, Psychiatrist.....as an example. This will do a few things for you. It will show that you are giving all the information you have, you are hiding nothing, your are willing to tell who gives you what and to keep from doubling up on meds, and that you are honest.
It also gives you something to refer back to when you get nervous. I keep the list as a file on my computer and print it before evey appointment for every Dr (using automatic update on the date). This way they have knowledge of everything and can make sure they do not give me meds that interact with each other. I will be printing one myself today for my PM apt. He's doing a RFA today and having my "appointment" too. I'm going to ask for a dose increase on my Dilaudid and I have to admit I'm a little scared too. I can't take just anything because of reactions and I do so hate for Dr's to give me new meds when a simple increase in the one that works would do just fine. I hate those med reactions too!!! So, I understand where you are.
Please know that, although the receptionist may copy whatever you bring in for your file, the Dr. will likely not read everything you bring in. Just have the one sheet of paper with a font big enough to read and give it to him/her, while keeping a copy for yourself. This paper will be the one you can refer back to. You may get one who wants to listen to everything, or you may get one who is a numbers guy/gal and wants "just the facts". Describing your pain is separate and individual and only you can do that in person.

Oh....I forgot to say good luck on your apt. Please keep us updated after it is over. I may or may not be able to chek on you today depending on the sedation for the RFA, but I'll check back soon.
