Chart, since you have proof I would go there in person with copies of my receipts, assuming its either a canceled check or debit card receipt. I would not pay for a service twice. I am hard headed, lol, however you have paid for the service. Dig your heels in but be civil at the same time. By chance do you keep your appt cards that you are given, I do, that too helps verify your appt dates.
Do not waste more money, a lawyer would charge you a lot more than $90 to send one stinking letter, trust me on that one.
You mentioned trying to obtain some records. Do they use a service that copies their records, a lot of drs office do & many will count the sheets requested & require prepayment for the records. It is legal to do this too. I would call the office & ask for medical records dept & see if they do their own records or have an outside service copy them.
If all else fails, on your next appt with your dr tell him what is going on & show him your receipts where you have paid. I would only do this as a last resort.
If you like your dr & this were me I would not fire him over this because I think if you have to discuss this extra billing with him he will have it taken care of.
I am having problems with the PM billing office right now. They are not billing Medicare first & of course the secondary is denying the bill because Medicare is my primary. When I called & spoke to a gal in the office she said oh you are mistaken on this. I said no, I am not mistaken, I am looking at these EOB's from the secondary denying the bill, plus I just got off the phone with them & yes I do know what I am talking about. Her tone changed & she said well I will call them & tell them to get it right. Guess what, Friday two more claims denied because of their ignorance. I will be calling them Monday, lol.

PS: Your EOB's from your insurance is your proof of your visits to that office. Get those all together for the past year.
