Follow up with forced med change

I finally called my doc.  He called back and talked with me.  He said this would happen and he is sorry it has to.  I have to deal with it until I see him next week.  If any of you do not know what i am talking about look at my post a few days ago "doc forced to change methadone".  I told him I am going through withdrawals and he said I should not because I am still on an "opiate".  He did say I could have days of feeling uncompfortable.  Its a pain in the back litteraly.  My pain levels are up by a few numbers, I am taking more b/t meds, bathroom problems, anxiety and feeling down.  He said it should gradually stop.  He also told me in our meeting next week we could adjust the morphine accordingly.  He also said if I need more b/t med to go ahead and take what I need.  He is a great doc that has his hands tied.  I am on 30mg 3x day of morphine cr.  Is this alot?  I went from 70mg methadone to this.  Is this a good conversion?  Does anyone know?  Are there conversion charts?  Is methadone alot stronger than morphine?  I just cant figure out why I am having these problems if I am replacing one with another.  I just hope my pain levels dont go off the charts.  Any comments would be appreciated.  Thanks and have a pain free day!

60mg Methadone = 150mg Morphine -- that said, keep in mind that you can't just start out at 150mg Morphine. Dosing would need to be titrated.

Also, people don't always need the exact equivalent. For example, when I switched to methadone from fentanyl, the equivalent dose would have been 30mg Methadone. But I can't tolerate that much. Probably over time we will work up to 20mg (that's what I am prescribed) but the side effects when you are starting on a new med can be harsh. If your doc is suggesting a med increase & you are in pain, maybe it's worth a try.
