I'm so sorry to hear about your pain! That sounds like no fun indeed! I have minor TMJ issues that fortunately are generally not painful, just annoying. I can't imagine having constant pain associated with it! I know money is an issue, and this probably isn't within your budget at the moment, but have you thought about seeing a neurologist? It could be more than just the TMJ that is causing your pain -- especially if you are having dizzy spells. Trigeminal nerve problems can cause widespread facial pain (although would not contribute to dizziness or ringing). It might be worth going to see a good neurologist who sub-specializes on migraine, trigeminal neuralgia, and other facial/head issues. They'd probably want some imaging study, but it is possible that they could pick up things on exam that a normal physician, or dentist wouldn't notice.
Have any of your docs ever tried you on nerve pain meds? A lot of antidepressant and anti-seizure meds are used to control nerve pain. Lyrica, cymbalta, amitriptylene, and gabapentin are some examples. Some of the older ones (amitriptylene, gabapentin) are cheap, yet can be quite effective in relieving or reducing nerve pain -- which is notoriously difficult to treat. I don't know how your GI issues would play into this though, and whether or not they would prevent you from taking these kinds of meds.
