Are you using ice well padded? If you are using anything that gets wet it can transmit too much cold also.
I prefer the ice bags I bring home from the hospital that have nice soft flocking around them. Plastic bags need to be wrapped in something like a double layer of dish towel - not the terry type, just the plain flour sack kind.
Nerves that were cut or injured - it's early for them to "wake up." They have to grow back. I have some that were cut in my neck a year and a half ago and haven't reconnected yet. I have abdominal areas that are still numb from surgery 2 years ago.
Susie mentioned inflammation. That may be why you are supposed to be using ice, although it seems a bit long. Remember to take it off after 20 minutes. I was allowed to use heat after a couple of weeks for my shoulder surgeries, just not real hot!
I had lots of pain in my shoulder and bicep long after both shoulder surgeries. It was coming from my neck. A steroid injection that went up to my head cured it accidentally.
When docs do shoulder surgery they anesthetize a big nerve plexus in between your neck and shoulder in front. Sometimes that causes pain for a long time.
