Dilaudid or Morphine, the short-acting drugs, (although potent), can be rotated to Buprenorphine as "Subutex" or "Suboxone" as long as the dose of Dilaudid you last had is run it's course and you almost feel the start of physical withdrawal like discomfort, mild nausea, sweaty palms, or just the feeling that the time to take your dose has come, (then wait a few hours to be safe). then begin the taper schedule maybe with extras like clonidine by patch or tablet, Vistaril or Phenergan as nausea and nerve agents. many people do this without any uncomfortable feelings at all. if you were dealing with Methadone or Fentanyl patch it is tough. in my opinion doctors that want you to go 3 days and have no opioid in your system prior to starting the Suboxone taper is crazy. in my area mental health doctors who offer detox for addicts do that. you are a pain patient who needs a different type of plan, pain and addiction specialist may be better. if you are still having pain don't feel pressure to get off the meds because you will open yourself to the possibilty of rusting and hardwiring of pain on the brain. also endocrine exhaustion. you will be shocked at how the Dilaudid can be discontinued without toture and at home.
