B, I can tell you first hand it's a difficult place to be where you are right now in making a decision. When I left my job of 20 years it was a struggle for me to make my decision. I did not go out of disability, instead I took a few months off. I then went to work at a smaller office, much less stressful. I went through the guilts & all that jazz. Once I was away from the office, I knew in 2 weeks I had made the right decision for me. Your situation is totally different than mine of course. But, don't let the guilt of how it will affect co-workers & such be a deciding factor. Your job will go on whether you are there or not. One of my regrets was I had a wonderful secretary, she had been with me for 4 years. Her & I had many conversations about this. She left not long after I did, lol. To this day we stay in contact with each other.
Who knows, going out on STD may be what you need for now. It will give you time to re-evaluate on whether going back is possible, or moving on to LTD.
Don't let the guilt trip you up, I did that for many years & should have left a whole lot sooner.
