ER visit for Hip/Groin/Pelvic/Low Low Back Pain

Well, I have posted about my ongoing struggle with IBD & trying to get a DX of CD or UC. I had went to the GI about 5-6 weeks ago & when I explained my pelvic/hip pain, she felt it was more likely due to IBD than the ovarian cyst like I had thought (even though the cyst is on the left, and most of my pain has been on the right). I was told by the GI's nurse that I would feel much improvement in my pain after a few days on prednisone. It has come to get worse & I have a feeling of pressure low in my abdomen. I kept bugging both the GI & CP about it and finally my PCP called me back letting me know that the pelvic ultrasound I had weeks prior (& had already received the results of [that it has grown, there may be 2 and we will do another ultrasound in 3 months]) was the culprit, the cyst had grown 3 times the size of my ovary & that I need to see a gyno specialist right away. I have been also having constant vaginal bleeding for the last 6 weeks, and now she mentioned a endometrial biopsy. So I figured this was the cause of my pain. The PCP said that the cyst may be leaking... I had been waiting & waiting for a call concerning the appt. and the nurse called the gyno telling them that I was in a lot of pain & the gyno told her if I am in that much pain I need to go to the ER.

The PCP had changed my pain medication from darvocet to tylenol 3s. (I was prescribed Norco [vicodin] before but when I started really having problems with my IBD before I was diagnosed, the pills were not digesting & coming out as sandpaper in my stool – very uncomfortable & uneffective). The T3s were not doing anything. I couldn't sleep & I was crying & crying. My husband said this is it, we are going to the ER.

So I thought maybe my cyst had ruptured. NOT. The ER doc said that ovarian cysts will not cause pain for this long, and that normally they cause no pain at all. He said that it definitely was not due to the cyst, especially because of the location of the pain being in my hip. He did say that if the cyst has grown, then I surely need it removed. He also said that the bleeding is a problem that needs to be corrected & I need to see someone within 2-3 days. But he would not give me a definitive answer to what is causing this pain, although he felt the IBD could be behind it or making it worse. He also thought maybe an issue with my uterus could be to blame.

BTW, both he and the nurse believe I have CD after reading my small bowel series results & listening to my symptoms.

Also, this was a much better visit to the ER than my last. They were so nice & caring & they took good care of me. I could tell that he wanted to fix me, but told me that wasn't going to happen today, but he would help me with the pain and make sure I didn't need an hospital stay. He gave me a script for liquid vicodin (which works pretty good on my pain - it doesn't get rid of it, but really makes everything tolerable) and did some lab work. He said I was slightly dehydrated (again!) and there wasn't anything TOO serious on my bloodwork to warrant a hospital stay (thank goodness), so home I went.

So, here's my question, does anyone else have this kind of pain with CD? My right hip is sooo sore & painful. It hurts to lay on that side. It's just horrible. I am just trying to figure out what is causing all this pain so I can eliminate it. I was slightly bummed to hear that it wasn't the cyst, because I know they are going to remove it due to the size & I had really hoped the pain would go away, too. All pain info would be greatly appreciated.

Gosh, this sounds a lot like what I experienced all summer long. Lots of right sided pain and vaginal bleeding. Crohn's has been under good conrol though, so I was confused about the pain. I used to get real bad right sided pain with my Crohn's when it was bad. My Gyno saw a cyst on my right ovary. The pain was really bad but not constant. Dr. gave me two birth control pills and told me to take 800 mg of Advil every four hours for the pain. Weeks went by with all this pain which now was also in my back and hip area. I kept calling the doctor and she seemed generally unimpressed/unconcerned. She told me that the pain would go away when the cyst burst. Anyway, long story short, the pain got worse and worse. Wound up in the ER after the pain was constant for about 36 hours and I could barely stand up straight. It was appendicitis and the ovarian cyst was hitting the appendix causing even more pain. So, they took both out.

No, I don't have any other joint pain. Yes, I am in a flare, I guess. I was just dx with IBD & they are trying to get it under control. Still losing weight & having problems, although the GI said the prednisone I am on would fix my pain problem, but it hasn't. I had also went & had an adjustment thinking maybe I was out of alignment, but that didn't do anything. It's just so confusing because although my doctor thought the cyst was causing this pain, it's on the left & the ER doc said that this right hip pain would NOT be due to the cyst. In my discharge papers, he did say something about watching for appendicitis. My pcp had me on progesterone to get the bleeding to stop, but that did not work. My GI said absolutely NO ibuprofen at all, so that is not an option.

The ER didn't do any tests or anything except blood work because I don't have insurance & I sure do hope that appendicitis is not what my problem is....

Broomhilda, as for the vaginal bleeding/clots/pain, see if your doctors would consider a uterine ablation instead of a hysterectomy. It is a very simple outpatient procedure and can cure all those problems. You cannot want to have anymore children though. It is where they go through your cervix and into your uterus. Hot water is then put into the uterus. Everything sort of gets burned away inside and then after a brief recovery time... no more periods at all. No more pain, no more bleeding, no more clots, no more tampons, no more pads. It's heaven.

Maybe you are right about needing a new GI–I called last week about this pain & still have not received a return call.... I think I am going to fax them to let them know about the ER visit.... I wish I could email.....

Heh, I get that too when dealing with different doctors for the same problem.  They ask me what's the deal with some thing that I wouldn't know and it's like I don't really understand, why don't you ask the other doctor? ;)

If it makes you feel any better, I had almost the exact same thing happen to me.  I had all sorts of girly pain and severe bleeding each month as soon as I was old enough to get it and they told me I'm just unlucky.  When I would have pain in that area outside of that time of the month they would tell me it was a cyst and I'll feel better after it bursts.  Not that they actually bothered to do any diagnostic tests, they just said it was a cyst and it'd go away and apparently I get cysts often and they burst.  Not so.  Turns out I had a huge giant fibroid tumor that was causing all the pain and bleeding.  I also had what they thought was a complex cyst, which turned out to be my poor ovary that had mutated into a giant non-cancerous tumor.  Since I had been blown off all the time for my symptoms they literally had no idea what was inside me until those things were squishing me so much that I was having trouble breathing and my blood pressure, normally 120/70 was constantly 147/121 and my pulse, normally about 85, was constantly 150.  Then they found those fun things and were like wow you need surgery.

I know the non-laproscopic surgery sounds scary, but if you have ever had a c-section that's pretty much what it is.  I haven't ever had children but they told me that's basically the type of incision they did for me.  My tumors was so large that I literally looked like a pregnant lady all the time despite only weighing about 100 pounds.  I was happy to learn that I had not gotten incurably fat from prednisone, and I look much better now since I'm not stuffed with tumors.  They only had to make a cut about 7 inches across on me to get those big huge things out, and it was actually the least painful surgical recovery I have ever had.  I felt much better literally as soon as I woke up from all the anesthetics.  I didn't even need painkillers, just ice sometimes.  It's not a very bad surgery at all, they make it sound so terrible but it's really easy and relatively painless.  All my girly related problems are completely gone now too, except for when stuff kind of acts funny when I'm on prednisone.

So I had the laproscopic diagnostic surgery & hysteroscopy D&C today & it was all for nothing!! The cyst was gone! So now I have the surgery pain AND the hip/pelvic/low back pain! I am so tired of this. I called the GI as soon as I got home & told them it was NOT a gyno problem. The lump & this pain MUST be from the IBD. And I am tired of it & want something to change. I am so tired of it. I don't understand why they can't help me. I don't understand why I keep going through all of these tests & crap & still hurt so badly without any end in sight. I am still on the prednisone. It's been forever it seems & that was supposed to help. All it's doing is making me swell. I am frustrated.

Have you been checked for Ankylosing Spondylitis the groin/pelvic/low back pain really is right on for A.S. I have A.S . there are a few peeps here with A.S. and Crohns.

Go to www.kickas.org its a very good support group I an there under another name. You can read up on A.S.

but were they looking for A.S. gene in your blood work are was this a regular CBC. A.S. will effect the ribs too.
