Hi everyone

I have a weird problem.about 3 weeks ago I developed pain on the left side of my anus,it came the same time as a spell of constipation that I can now say is on the decrease.In all the 20 years of CD i have never been constipated,my problem is always trying to hold long enough to reach a toilet,not always succeeding.I prefer D over C anyday.

As to the pain in my anus,at night it gets so bad that I can't sleep,it stretchs right into the left butt cheek,sometimes just making a slight movement causes me pain.

I'm seeing my GIT doctor 2morrow,any advise is as always appreciated

I wonder if you are developing a fistula. I would definetly bring this up with your GI today.

Hi,

Sorry to hear you are in so much pain. Maybe you have a periannal abcess. I am glad you are going to your GI. Take care and let us know what the doc says.

Just an update on my situation
Saw the GI who said it was an abscess,put me on flagyl and cipro 4 two weeks. On followup told me to continue flagyl and cipro for another 2 weeks and put me on steroids 60mg.Will do a colonoscopy once the abscess has settled down a bit to see what's going on in there

Hope the medicines help.... Butt pain sucks... *Pain, in general, sucks... but... man... It's like... You tell someone 'this is a pain in the butt', and then you have to stop and think 'Do I mean metaphorically or literally?' Because either one could fit...*

salt bath, no soap, olive oil afterwards if u can tolerate it, then do salt bath again, table salt is ok if u dont have epsom, pour it in

Personally, I don't think that the antibiotics will do anything for your abscess.  I was in the hospital on high doses of iv antib. and my abscess kept growing.  An abscess is a walled off area of infection, so the antib. can't penetrate it. 

One thing the antib. will do is help the inflammation that you have going on in your rectal area.  Flagyl works really well for me there.

From my experience, the abscess is going to get bigger, and the pain is going to get worse until you have a colorectal surgeon drain it and clean it out and leave it open to heal from the inside out.  This needs to be done in an operating room, NOT in the office.

I agree with everything Chroniemomx2 said. Antibiotics are very unlikely to work.. The sooner it is opened and drained the better. The longer you wait, the more you risk getting a fistula and you really don't want one. (if you do want one, you can have both of mine!!)

I would just add that you need to see a colo-rectal surgeon that is experienced with CD. Not a little -- but experienced. The treatment of abscesses and fistulas in CD folks is different from non-CD. Honestly, the fact that your Doc said to just use antibiotics really makes me think your GI isn't very experienced either -- at least not with CD peri-rectal abscesses.

Wow - I know what you're going through.  I had my perianal abscess drained a couple of weeks ago.  It was so bad that I didn't have enough energy to stand up long enough to wash my teeth at the basin.  Antibiotics may help clear up the surrounding area but there was no way mine was shifting without surgical incision.  The bad news is that having it packed afterwards isn't fun (but is more bearable than I thought it would be)but good news is you will feel a milion times better after the drainage and all that nasty poison that is infecting your body comes out.  I was in hospital a week after my drainage having antibiotics but slowly and surely getting my life back, I'm feeling like my oldself again.

Thank you all so much for your replies (and prayers and good wishes) we really all need those.Just an update, I have finished my flagyl and cipro treatment and am now just on the pred 60mg.I called the Dr yesterday and he told me stop the anti-biotics,even though I still have the pain in the ass,though not as bad as before. He also told me that once things are settled ,to do a colonoscopy in 4-5 months time.He told me that the pred would clear the inflamed area.I'm just concerned that without the anti-biotics the abscess will start acting up again.

I've been on the pred for 2 weeks now and my appetite is back and i'm eating a lot more,but it is not reflected on the scale, I still weigh the same as when I started the pred (60 kg).

This is such a great place for all of us ,the support is amazing,just to know there are people out there sharing similar experiences makes the burden bearable.

I'm in the same situation atm, but in a more progressed state.

I had to go for surgery to bust an abscess that had grown the size of a snooker ball in my perenium area (between the scrotum & anus). What's more it has a connection to a fistula that connects to the colon running across my right buttock cheek, i can currently fart through the open hole they had to leave (stitching it up would add a repeat of the abscess). Just been for an MRI today to see what course to take next. If you feel like passing wind is going somewhere it shouldn't- take note!

Prior to this time i had a discharge for 3 years from the rear which was noted & treated with drugs. Probably best to nip things in the bud early and see if you can prevent such a thing happening rather than cure it. See if you can request to see a Colorectal Surgeon through the GP if you get no joy elsewhere. (This is what i ended up doing)

OK this will sound wierd and gross, but I inspect myself for fistulas and abcesses. This isn't terribly difficult to do if you're a) familiar with what they look like and b) willing to contort a bit with a small flash light, a towel and a shaving mirror.

To find out what they look like, do a google image search. Yes the images you see will be disturbing.

To inspect yourself, if you're still up to it...
1) barricade yourself into the bathroom with the flashlight, shaving mirror and towel.
2) Drop your trousers and underpants, budle the towel a bit on the floor.
3) Squat over said towel, and position the shaving mirror on the towel until you get a clear view of your anus. (Gross, yes, I know...)
4) Turn on and aim the small flashlight at your anus.
5) Mentally compare notes to what you saw for fistulas on google images and what you're seeing now.
6) Touch it... CAREFULLY! If it stings, it could be one of several things, make an appointment with your doctor ASAP. Since you said it already hurts, it will likley sting.
7) wash your hands, tell no one you did this, until you're in your GI doctors office... then spill the beans that you followed some crazy mans advice... Tell him/her what you saw...

There are things that a doctor will look for when trying to tell the difference between abcess and fistula. Don't try to diagnose *what* it is. Just figure out where it is and a good way of describing it, pain too if it's *unusual*. The sooner abcesses and fistulae are caught the easier they are to treat. However, unless pointed out when in their earliest stages they can be easy for a trained eye to overlook.

I've had two confirmed this way, I'm looking at a third confirmed soon. (I spotted it last night.... similar symptoms to your own...)
