I had an abscess in July 2010 drained and it unfortunately came back in Jan 2011 and needed to be lanced again. After drainage, there is always a chance that the fistula that caused the abscess can heal on its own. However, in my case the fistula remained and I would get a slightly painful "pimple" head where the lancing was done. If this happens, there are a few options, which you should discuss with your doctor.
My only advice to you, if this thing comes back, is to see a doctor that specializes in these things. I believe a Colorectal specialist would be the best to make any decisions. I had the fistula undiagnosed for over 2 years because I went to my normal doctor, who thought it was just "taking a while to heal" then went to a General surgeon, who thought it might be a fistula but wasn't sure, to a Colorectal surgeon who gave me the honest answer "if it keeps coming back and bothering you, what we should do is an examination under sedation so we can see what this thing really is."
