Your age wouldn't come into play with a recto-vaginal fistula caused by Crohn's disease. There are some fistulas caused by tearing during labor that has nothing to do with Crohn's, so you can rule that information out.
Yeah, you would know the difference between blood and fecal matter. When I first suspected that I had one, I carefully cleaned the "bottom" area and inserted a Q-tip into the vagina. The fecal matter showed up on the Q-tip. I was eventually able to pinpoint it by doing this. I went up left-side only, then right side only, etc. using a clean Q-tip each time.
I would recommend a gynecologist, though finding one that is educated about Crohn's disease and fistulas may be a challenge. Regardless, talk to you GI. A fistula usually signals disease that's more active than he/she is aware.
