I have UnitedHealthcare and I am enrolled in the Remistart program. I've spoken on the phone with them several times, and they are very helpful. They would be able to tell you if you're eligible.
The program has restrictions as to who can receive assistance. I copied the paragraphs below from their website. I think you need to have commercial insurance.
