Could it be a fistula? I am a Female w/33 yrs worth of crohn's. What or who should I see first ?

A few months ago i was having lots of vaginal infections ...been on antibiotics and for the most part got about 90% better ...didn't bother going back to the gyno just dealt with it .

about a week ago i noticed some blood drops on my under-ware ....
I don't have any pain from it : but the blood caused me to investigate and sure enough there is an abcess between the rectum and vagina on the outside ...which continues to let out drops of blood throughout the day ...
It has been so very long since I have had one of these so began to research ...that's when i got reminded that it could be a fistula.

My question is : the fact that it is bleeding ; is that good or bad ? My GI pretty much doesn't see me anymore since my Cirrhosis diagnosis. The only time I see him is when it is time for a colonoscopy.
I do not like my GyN at all ... so if that is who I need to go to then I start the search for a new one.
This sounds silly ...i know ...but who would be more responsible for this ?

The GI or the GYN ? Who should I go to ?

Also I have been experiencing a greater than normal amount of arthritic pain and tiredness ...also in the last week I began to develop headaches sometimes pretty severe ..mostly at night ... I NEVER GET HEADACHES so this is really unusual - I don't know if any of this is related to the abcess ?

    You need to see your OB and your GI about the same time. They will try to bounce you back and forth between the two a bit. I would also get a referral to talk to a colo-rectal surgeon. The GI will know not to treat you with just antibiotics, which an uneducated OB might try to do. If you don't like your current OB, get a new one fast. As for headaches, that could be due to the infection. My last peri-anal abscess went septic within 36 hours of the lump forming. It actually might be worth going to the ER about, especially since it's bleeding, which means infection can get into the blood stream easier. I'd be less worried if it just oozed puss or something. Make sure if they cut and drain the abscess that they teach you how to pack it with gauze so that it heals correctly. If not, it can turn into a recto-vaginal fistula (if it hasn't already). That's what happened with me.

    Sorry this is a little scattered, I had a baby 3 weeks ago and am a bit sleep deprived!

    Did you go see the doctor and get some treatment? As scifigal2k posted you need to see the two doctors at the same time if possible. Fistuals can become nasty if not dealt with in a timely manner, so I hope you got that looked at. Take care and there is always a lot of help here in these forums.
