3rd Fistula surgery: 2nd Filac Surgery

Kerry90---I'm in Canada too and have been fighting to get any kind of fistula surgery for well over 2 years now. I've found surgeons here to be discouraging and far too conservative. It's been a horrific ordeal for me.....I will be travelling next week to see a surgeon to do a stem cell transplant. Failing this, I have a name of a surgeon in Texas and will contact him to see if he will see me.

Dimitri--- are you doing the laser? Did you happen to find out the name of the "paste" someone else was referring too? Thank you.

thanks Dimitri---I see the surgeon next week but I'm reading that a "side effect" is abscessing which freaks me out. I thought the point of the stem cells were to rid the body of infection and close the tract. I'll hear him out but I don't think I'll do it if it involves setons and in increased risk of abscessing.

My after-wound almost closed up,still a few days to go,but the going is not very good unfortunately.

I saw minimal pus gathering in the wound in the last few weeks,everyday,like always did before the surgery.Now that the wound is almost closed down I feel pressure and pain in 9 o clock.Where the lateral side track was seen in vaaft.

In the last operation surgeon layed open 12 o clock deeply where the seton was(and all the operations were done) and saw the lateral side track and curated it.And it was minimally cut when I looked at it,my guess is he only filac `d through and cut minimally.Surprise surprise,9 o clock has the show floor now.

My guess is that the fistula will either make a path to external through the path of least resistance now that the wound is closing up or it wont be able to do that and will turn to abscess or make way another way.I think it used to pus out through 12 o clock through the connections,as there is pain and pressure now I can not say for sure if its gonna be able to make a way out.If it comes to abscess level pains I will go on amox-clav..


I am also seriously thinking having kshar sutra at india having read a lot of testimonials from canadians and so on.It seems like it is basicly cutting seton with some antiseptic coating and the doctor they are suggesting sound legit.

I will try to bear the pain for a bit to see if it can make a path out,so I have some proof for the surgeon smtng else than pain.Also without an external opening and lack of abscess,how will they be able to find the track?Maybe an endo-ultrasound might help.

Thanks for the well wishes and the link Dimitri,it really looks like a solid paper,and the surgeons knew what they were doing.I will take into consideration when the surgery time comes,the guy on the paper became a proffessor and has a good reputation as well.They seem to be wrapping elastic band to the cutting seton to lessen the excessive cutting as far as I can understand.

I contacted my csr and sent him some photos with pus visible and report,he said this is usual and expected and we should wait it out for a few months.

There is still 4/10 pain when sitting and a bit pus collected every few hours in the open wound.The wound is almost totally healed and I am curious what will happen when it closes up totally in a few days.

As far as common sense goes,if there is pus coming out then it is not healed and will not heal by itself.I suspect there is an unhealed fistula at 9 oclock internally which was going out at 12 externally before.Horseshoe is that I guess?I hope it doesnt make additional tracks now the external opening is closing up.

Before cutting seton, for me what seems to be needed first of all is a good imaging of the tracks,as missing a bit makes it repeat again and it seems like surgeons and mri kept missing the 9 o clock for me,this csr saw it with vaaft but probably the curation with vaaft/laser was not enough as you mentioned..12 o clock was layed open hugely 3 times now,that makes me think the culprit is not that area at this point.

I read that kshar sutra indian surgeon was able to find tracks without an external opening for two canadian guys in their testimony.How would he do it though?I know it is possible to find the internal opening by hand sometimes,maybe endo-ultrasound as well which has as good results as mri..Another technic widely used:Without an external opening how would the csr insert blue ink and take an xray/ct?

After finding the track the cutting seton would be inserted accordingly I guess.Otherwise it is just guess game as well.Anyway,I will wait a bit to see what happens down there and accordingly.
