I do what patientspiders does....
except I buy mine at our medical supply store and I get 200, 12 ply 4x4 gauze pads for around $6. I take 2 of them, and fold them in half and tuck them right in my butt crack. I have a fistula next to my anus, so that is why I use them. But also, I have the leakage from my anus after a bm, so that helps that also. I have absolutely no irritation from the drainage, and I just change the gauze whenever I use the bathroom.
As far as the rv fistula goes....when I had mine, my gyn. told me to put crisco on the area down there....not on the fistula but close to it, and above it to preserve that area down there. I cannot tell you what a difference that made. It was getting raw when I saw him, and then I started doing that, and it healed and never got raw again. I wore a pad to catch the drainage from the rv fistula, and mine was a large fistula. He had me blow dryer the area on cool after my shower in the morning and then apply a layer of crisco, and it was good for the day. I do recommend buying separate crisco, and not using the jar you cook with. :)
