Tookie, Mycobacterium avium Paratuberculosis aka MAP is the bug that is front runner in the suspect list for causing Crohn's Disease. If you want to know more, go to YouTube and type the name into the search button. Personally I feel sure it is the culprit.
But all that is irrelevant to you right now, because there is no known, let alone proven, therapy against MAP that would provide an instant miracle cure for your problems. All you can reasonably hope for meantime is symptomatic relief.
What you need to be doing right now is what the other folks here have said; devote all your attention and efforts to finding a doctor with experience of this sort of problem. A colorectal surgeon specialising in Crohn's may well be your best bet, as many GIs would not see a rare occurrence like this often, but if it ends in surgery (as seems likely, especially if it is a fistula) then the CR surgeons will be the ones who deal with all such cases like this.
Don't take no for an answer, get all the different scans you can, and get referred to someone else every time a doctor says "I don't know". You can't blame them for not knowing, thank God (and them) for their honesty when they don't pretend they do know - but somewhere there will be a GI who has seen this, and you need to hurry things along as it could take long enough to find him/her !
