Lots and lots of unbearable pain...I was perforated when I had my first surgery and was diagnosed. If you're in enough pain that you think it's possible for you to be perforated, please go to the ER.

And - in no time at all w/a perforation you will spike a temperature as the infection spreads thru out your abdomen. Perforations can quickly become killers.

Is this after a colonoscopy? After my last colonoscopy, I was told to seek help immediately if I passed more than an eggcupful of blood or experienced severe abdominal pain. I don't know if that helps, but I agree with the other posters; if you think you have a perforation, then don't wait.
