Fistula question for the ladies

I've been having some severe pain around my genitals lately...it started off with what I thought was a UTI, so I took some left over Cipro my doc prescribed a couple of months ago to see if that would help. If anything, it's gotten worse. The pain is more centered around my vagina, and not my urethra so much anymore. The area is inflamed, and it hurts ten times worse with a bowel movement...Do fistuals/abcesses happen in that area? Is it something I should call my GI about, or go to the ER?
Also, I've had dark yellowish stuff come out while wiping that area, and also very small amounts of blood.

I'm sorry for being so graphic. I'm just kind of afraid but want to avoid an unnecessary ER visit...I'm terrified of catheters.
