LDN: oral or transdermal?

After not getting better on Pentasa (and Pentasa and Entocort) with mild colon and ileum inflammation after scope last week I think my doctor may actually let me try LDN. Would anyone recommend one delivery type over the other?

On another note I don't know what the scope did but I got orders of magnitude worse immediatley following it. Trips with just blood and mucus, pain, increased frequency, etc. After four scopes this is the first that has caused a worsening of symptoms :(

Everytime I do a scope I feel horrid for about 2-3 days afterwards, The blood could very well be from biopsies taken.

As for the LDN, I have not used it myself, but read that some others have had issues with the oral becuz of the fillers etc, but did much better on the transdermal.

Hopefully someone who has more experience will share their thoughts with you soon. Hugs!

Nanners is right about the fillers being a potential problem, have your doc prescribe it w/out lactose as a filler if that bothers you. I get mine from Skip's pharmacy in Boca Raton Florida. I take the capsules rather than the transdermal cream and it seems to work great! Skip's seem to really know what they are doing and will mail your prescription to you, no problem. Have you been over to the UC forum? There are lots of posts about LDN there. Good luck, hope it works for you!

Thanks guys. Yeah, I'm going to try to read two of the long threads on it today over at teh UC forum. THought I might get a different response here so I checked just in case. I hope the doc hasn't changed his mind. At first he was very reticent, then he said he would "muddle through it with me." We'll see how it goes!

I don't have a problem with lactose. I drink an Ensure everyday for a snack. I have a problem with the sugar but I need the calories.

I have been on LDN since the end of June 2011. It is working for me, but it took awhile to find the right filler for me. Talk to the pharmacist and find out what they are using!! for me the filler Avicel, which is a kind of cellulose,  was the worst one. You can find info. on line regarding the many filler available. At this time my pharmacist is using acidophilus (a probiotic) to fill my medication. Good luck!!

There has been a lot of talk about LDN on the UC board in the recently. I believe I heard that transdermal isn't proven to provide benefits but a few members were having good luck with it. At the same time, it's difficult to tell what you're having luck with when you're restricting diet and taking multiple medications. I was flaring one day and the next, I was in remission for absoutely no medical reason. Things are so unpredictable.

Here is the post I saw that made me question whether transdermal would be effective:

Just been reading up on emails and saw this post. Michele the reason why you may be finding LDN isn’t working for you is that Dr Zagon has recently conducted a study about the transdermal cream and it doesn’t enter the body systematically. This means it only has an effect on the area you rub it in, hence it is now highly regarded for wound healing. Before giving up on LDN, I would as Mara suggests perhaps trying the pills at 3mg. Here’s the email from Dr Zagon:-

Quite simply, naltrexone does not have the physicochemical properties to penetrate deeply into the skin and to enter the systemic circulation. And the systemic circulation would be the source for the entire body. In fact, in our paper in press with Experimental Biology and Medicine we cite some of these papers - and we ourselves found that naltrexone in a cream form ("transdermal") does not alter cells in the skin distant to where we have made our application. So the bottom line: at this time I am not aware of a transdermal cream of NTX that enters the body - much less with the pharmacokinetics of LDN (see below).

Now, many folks are out in the world looking to develop an enhancer for naltrexone penetration - the market for drug abuse is huge.

My final remark about this is that even if one does find the magic enhancer, and naltrexone does enter the systemic circulation, then a very specific amount of naltrexone in this enhancer/transdermal delivery system needs to be found that mimics LDN - blocks receptors for 4-6 hours and have a remaining 18-20 hours for the upregulated receptors/peptides to interact and cause the good things needed. Anything less than that can be catastrophic in the sense that some biological systems would be exacerbated by the failure of having opioid control mechanisms.
