How can you tell whether it is proctitis or hemorrhoids??

I have always thought I had only proctitis along with anal fissures and these were the cause of my rectal pain. However, in the last scope, my GI told me that I have roids, too. How can I tell the difference? Both cause rectal pain. But the pain of the proctitis, IMO, is like stabbing and I have swelling. Also meds differ: pentasa enema or a cream. I'm confused and I don't want to leave these roids untreated until they become worse. I hate surgeons. Before the colonscopy, I saw two of the well-known ones in my city and they could not see the roids. Any advice?
