Topical Metronidazole for treatment of perianal disease

Does anyone have experience using topical metronidazole to treat symptoms of perianal disease?

I am struggling with a bad flare as Humira has worn off (stopped late-March) and Remicade has not given me relief for more than a week (started early-April, only 2 infusions so far, next one - a double dose - scheduled in 2 weeks). Doc has also put me on Canasa to help calm things.

Cipro and Metronidazole by mouth have destroyed my stomach in the past, so I have been reluctant to go back on them, but I have yet to try the topical variation.

What do you mean by topical? Sorry I didn't get this. Do you mean rectal in the form of suppositories. Well, I was told that they are better than oral ones, esp. flagyl in treating rectal problems. But how did metronidazole and cipro upset your stomach? I have a real problem with flagyl and whenever I have to take it to treat a proctitis flare up, I have to be put on Zofran daily im injections during the whole course of the antibiotic. Flagyl causes me nausea and vomiting. But Cipro is very gentle on my stomach though I got GERD, gastritis and duodenitis. Do you have stomach problems?

Topical, meaning it is in a cream/ointment form. It was made by a compounding pharmacy. If you google topical metronidazole there are a couple studies that have been done in the last few years that show decent benefit from its use.

My Crohn's usually doesn't negatively affect my stomach, but antibiotics, especially cipro and flagyl absolutely destroy my stomach and appetite and end up resulting in diarrhea and a significant increase in BM.

This topical form of metronidazole is supposed to give the benefits of the medication without the side effects that can affect the stomach.

Just curious if anyone else has used it in this form before. Not sure how popular it is in the US since it can only be created at a compounding pharmacy (and I was surprised to find one locally).
