I have severe anal fissures. They are v itchy and painful and i apply every possible cream and lidocaine I find but in vain. Add up I have nonstop, urgent d. Lomotil and imodium lost their effectiveness. I started resuming eating but could it be the reason. Two weeks ago when my GI knew that I have d, he told me to stop steroid enemas. I don't take pentasa enema regularly either. What could be the reason? I'm afraid if I see my GI he might put me on pred again esp that intestinal pain returned. my previous experience w pred brought me hypertension. Any suggestions?

Prednisone is not the answer for anal fissures. I think you mean that the prednisone should stop the diarrhea?

Have you been taking sitz baths to help heal the rectum?

I don't know if this will help, but I find that aquafor or vaseline is a good way to protect the delicate skin of the rectum. Apply it after every use of the toilet and after a sitz bath.

The reason to stop steroid enemas is because they can thin the tissue of the rectum and make these problems worse with long-term use. Pentasa enemas will not do this.

Thank you, Kazbern. Well, things are becoming worse. I use the bathroom 3 times in 10 minutes until lomotil kicks in and this is horrible w fissures that are bleeding. I can now see red spots w bms. I kept rushing to every toilette I found at work today. It was so humiliating.
I doubt it's a normal flare up. I'm suspecting that the course of conaz (tinidazole and norfloxacin) I finished 2 days ago caused this crazy d. I had before c diff. This time I'm not vomiting incessentaly as i did last time and i do have abdominal cramps but they are not that severe but d is literally crazy. As I'm fortunate, I can't find someone to consult as my GI is travelling.
Thanks for the advice of using vaseline. I never tried it before.

If you think the diarrhea is primarily from using the antibiotics, then pred is not the answer. Keep using probiotics and eating yogurt with live cultures and be patient - hopefully it will pass.

Probiotics and live culture yogurts always increased my d. I took I modium after Lomotil. The problem is I got hit by a diarrheal bout for three successive times in 10 - 15. Then d rests but this deepens the anal fissures and the pain became extremely horrible. Simply I'm crying and screaming. the diarrheal bouts recur every 2 hrs and Imodium and lomotil lost their effect.
I called the GP and she suggested I take sulpha. I'm afraid this might screw things up.

Minnie, please be patient and let your body heal from the drugs you are putting in. The lomotil and immodium then followed by more of whatever (sulpha? really???) - next you'll be constipated and complaining of an impaction and we'll be repeating the whole saga. I'm sorry that you're having such bad diarrhea. Get some epsom salts, put them in a warm bath and sit there for 15 minutes. Repeat as needed. Put vaseline on your bottom after you use the toilet. Don't take any more meds unless they're anti-inflammatories meant to treat your IBD.

Kazbern, thank you very much. I too am afraid of taking anything more but d won't stop. I need to have a bm but I can't imagine the pain that will follow.
Sorry for this question. But is vaseline like the one we use on our body or is it special for rectal use?

just the same one you use on your body. It's good for a lot of things.

Your diarrhea will eventually stop.

After I had a baby and had significant tearing, the doctor told me to spray a water bottle "down there" filled with warm water while I was urinating, this kept my tears and stitches from stinging when I went. I did the same when I had a bowel movement and found things a lot less painful. It also reduces how much you have to wipe. This may help decrease the pain while you have a BM, at least until your fissures heal. Good Luck!

Fissures can hurt a lot.

Here (Brazil), most toilets have a little shower just besides our "sitting" place". It cleans all out and I just have to dry the area.

In portuguese is "ducha higiênica" something like higienic shower? You can google it... and meybe have it installed for you.

When I travel to US I find it hery hard to lean just in wet wipes.

Thank you, Djin. I'm not a US resident and I do too have this toilet shower where I live. The problem w regular water is that it makes me feel that my butt is on fire the minute it touches it. I'll try warm water as was suggested by Myrne.

I hope you're doing better now, minnie.

I had what I think were anal fissures while I was in hospital - at first I put it down to soreness from wiping too much, but as the BMs decreased and the pain got worse, I started to wonder... Also had blood flecks on the toilet paper.

I just became very, very careful, dabbling at my arse with moistened toilet paper as though it were made of the most precious and fragile material on earth. Absolutely horrible experience. I really would try the sitz baths, warm water bottles, etc., rather than go through that pain. This next bit is going to be unavoidably revolting, but if going in a sitz bath is less painful than going in a toilet I would do it.

NCOT, sorry that you are suffering. This dd likes to entertain us - lol. I would def try anything other than fissures.
