Is this going to be the norm?

I posted last week asking about a treatment for explosive watery d. Many fellows suggested I had a c.difficile test. I did and it was negative. For the third week in a row, I had the same diarrheal bouts. I woke up today w severe watery diarrhea and my trips increased so I took Imodium. I had to rush to my GI's appointment. At the clinic, the trips increased despite the Imodium. I nearly fainted. When I told him, he was not that impressed and I could hear him say thu he didn't in reality "it's normal". He said sorry, take Imodium whenever this happens again. He asserted that I can repeat Imodium every 4-6 hrs. He prescribed Rifaximin thu I told him I failed it 3 times before but he insisted and for the diarrhea I have to take Questran. What baffled me was his reaction towards my blood work. I was happy like a baby that my WBC was 6.9. He said that Imuran could now only be upped to 175mg and Entocort won't be tapered at the time being. We still have to wait until Imuran makes more effect. My anemia can wait at the time being and my GERD symptoms can be solved by eating small meals.
My question is: Will this type of explosive watery d persist? Should I deal w it as the norm? D affected my submitting a paper for my PhD as it left me drained and I could not think properly. I had to delay it for a day but my supervisor did not consider it for revision as it was to be handed in to a committee. Is my lovely Crohn's going to ruin my career? I'm a college teacher and pursuing my studies is not a luxury. GERD has never done this to me thu I got it when I was at college and I used to throw up 13+ times /day.

Gumby, thank you very much. Well, I can't be put on biologics due to insurance reasons. Yes, I told NCOT several times that I, like her, respond only to steroids but these steroids are affecting my eyes. Vicious circle, right? I'll try Rifaximin for the 4th time and hope the mixture of Imodium and Questran brings diarrhea to a halt.

NCOT, thank you very much. The first 2 scopes back in 2007 and 2009 showed mild active ileocolitis. However, symptoms got aggravated since 2010 and that's why I was put on pred. Being on pred followed by Entocort shows only active colitis and proctitis. These scopes were in 2011 and 2012, respectively. This could be a reason why he doesn't want to prescribe biologics. I think this watery d might be caused by the excessive antibiotics I have had over a month (they saved me to have the perianal abscess cut open) and that's why he prescribed Rifaximin.

I've never heard about these terms "localised or extensive". The problem is that when I stop meds, diarrhea becomes uncontrollable. In 2009, I was extremely depressed and decided to stop meds abruptly. At that time, I was on pentasa 2g, dexamethasone injections and I had just started pred then to treat a pseudomembraneous colitis bout. I was keen to take the injections but refused the pentasa and pred and these were the worst 4 days in my life. I've never felt such excruciating pain like that one. Also, before I started any of the Crohn's meds, I was being tortured. My GI thought then I had a gynecological problem and refused to name the pain and weight loss as "colitis". Only tests and exams made him rule out any gynecological origin for my pain and eventually he got convinced that it was colon-realted.
Deeply inside, I believe I have indeterminate colitis thu my GI said it was Crohn's. All symptoms conform more to the indeterminate classification. I have: proctitis, tenesmus which agree w UC but at the same time my pain is centralized in my intestines esp. the TI, got non-healing fissures and had a perianal abscess which goes w CD. A friend took one of my biopsies by mistake to Australia and asked him to re-analyze it and the result was UC. However, my response to Entocort which is only intended for CD leaves me baffled. I don't know. I'll try to be an obedient patient and follow the above mentioned plan to the letter w/o screwing it up.

Vitaka, I really appreciate your post. I'm really impressed by your perseverance. I still consider myself a newbie (only 6 years w this darn disease). May be I was not clear. I got embarrassed and did not tell him I was sick this time. I assumed that if I sent him the paper before the committee was held, he would look at it. I was mistaken. You are definitely right. I need to talk to him openly about my health issues. I told him before that I have a chronic disease that puts me into flare ups but did not specify what this disease is. I didn't want to sound as a person who pleads for an extension because of my sickness.
Actually, when my stomach used to give me really hard time, still I was first of my class and during my masters studies, I used to get fellowships. So, I won't feel embarrassed if I tell him about the truth of my disease esp.that he thinks highly of me and he pronounced this to my other supervisor and I was present in their meeting. May be he does not figure out that I have such a debilitating disease that can affect my thinking when in the midst of a flare up.

Really, I don't know. I was told before that I have IBS in addition to IBD which I'm sure of. No, my disease is not severe but I've undergone all possible tests but nothing else could be confirmed. When biopsies are analyzed, they tend to show features pertinent to IBD either UC or CD which means my diagnosis is mid-way. My GI has told me 6 years ago that my disease would develop by years. I think he was correct. I stopped trying to classify it. IBD has got 8 subtypes. All the 8 in the end are treated the same way. Sometimes I keep wondering why are doctors that keen on labelling my disease. My GI doesn't do this but at least others whom I've consulted for a 2nd opinion do.
Maybe what makes me more of a Crohn's patient are my symptoms which almost disappeared except for diarrhea. When I started my complaints I used to have severe abdominal pain which no painkiller could control and the onset of pain was sudden and was simultaneous to elevated inflammation markers esp. calprotectin in stool. These markers declined after being on pred. It was only after I was put on pred that things became bearable. Fever, sudden weight loss and even diarrhea were other manifestations.
I've never experienced these severe symptoms that a lot of fellows mention in their posts. My disease was quiet or as we would call it "mild". But there are a number of factors that made it quiet. I boycotted high fibers all together 2 years ago. I know that number one enemy for me is fats so I try to avoid it as much as possible.
My GP blames Nexium for my diarrhea and I would add Imuran as another culprit. But things were not like that in the past. I stayed for 29 years severely constipated. I could go for 14 days w/o having a bm and of course I exhausted all types and forms of laxatives w/o any avail. All of a sudden, this C turned into D. Accordingly, my GI treated me for a year w antibiotics thinking that the flora might have changed. BTW, I was not on Nexium then. However, symptoms deteriorated and fever and weight loss became more aggravated, Sulfasalasine (failed) and then Pentasa (which were added then) failed to control symptoms. At that time, I was studying for my masters and I'm known to be anxious by nature, s my GI attributed this to stress. Only after I finished my MA in 2009 and the persistence of my sypmtoms and the
excruciating pain did my GI become convinced that there is something else other than IBS that is causing these symptoms.
Sorry for this quite lengthy post but I was telling you abt my history (if you might be interested); a history I failed to understand. And if I use flashback technique to look at my childhood, I would def find incidents that would suggest I had problems in my GIT since my early childhood.
