Internal Rectal - Vaginal Fistula - ISSUES

I thought my fistula was healing, but I guess I was so inflamed that it was just swollen shut. 2 weeks on IV and oral antibiotics it is wide open. Since I have diarrhea all the time I have leakage issues. To the point that it wakes me up at night. I do what I can to keep as clean as possible, but the tissue in that area is very irritated and I am not sure what else I can do to avoid infection, irritation, itching and swelling.

I have asked my GI & OBGYN and they are both dismissive about it - like its no big deal. they aren't leaving with it - I am!!! and its a big deal - just ask my poor husband :)
