Clear Colonoscopy - what now?

A month ago I was admitted for an urgent colonoscopy due to rectal bleeding, stomach cramps, bouts of constipation/diarrhea. My GP suspected IBD. Blood tests showed anemia but otherwise normal. I was put on a low reside diet, which seemed to control the bleeding. At the colonoscopy my Dr reassured me that everything looked OK, that he suspected IBS but would review in clinic appointment (I'm still waiting). However, two weeks since, after introducing some of the foods back into my diet I have had a 'flare up' of cramping and rectal bleeding again. Any ideas? Now the GP says that bleeding doesn't tally with IBS. Is it possibly to present a clear colon and have IBD?

You need to wait until you get the results of your biopsies taken during the scope. You should also realize that the colonoscopy does not clear the entire bowel, just the large bowel. I would push for a pill cam or more imaging before dismissing IBD. Bleeding is not consistent with IBS.

Thank you for your reply. My General Doctor is trying to chase up the results to find out what is going on. I find that I am wiped out during the flare ups, and understandably bleeding can be disconcerting.

If your GP is gonna fob you off with IBS, then press him some more about the bleeding. It has to be coming from somewhere. It could be haemorrhoids, but I would've thought the colonoscopy would have picked up on those.

With Crohn's, it's possible to have disease in the small bowel higher than a colonoscopy can reach. It's also possible you went into a temporary remission. IBD can be a bugger to diagnose for some people. If I was your GI, I would retain a suspicion of IBD in your case. Hopefully the biopsies will yield more info.

Thank you for your response. According to the GI the biopsies came back clear. The GI has now referred me for a sigmoidoscopy to have a closer look at that part. I don't know how to interpret this as wouldn't a colonoscopy have picked up on anything a sigmoidoscopy would?

Yes, unless he failed to take adequate biopsies there. Time for a phone call to discuss.
