I would have hoped your doctor would have given you a chart to follow as you decrease from one med and start on another. Both meds you can take together as long as it is prescribed by your doctor. Personally I hate Dilantin as I drug-studied that medication long before it came out on the market and the only thing it did for me was make me gain weight. But that's my own opinion.
It is possible to have seizures or a seizure and not be epileptic. Sometimes they can be brought on by an intense fever. Topomax shouldn't induce a seizure as it is an anti-convulsant.
Here is some info on Topomax aka Topiramate: Topiramate is used alone or with other medications to treat certain types of seizures in people who have epilepsy. Topiramate is also used with other medications to control seizures in people who have Lennox-Gastaut syndrome (a disorder that causes seizures and developmental delays). Topiramate is used to treat patients who continue to have seizures even when they take other antiseizure medications. Topiramate is also used to prevent migraine headaches but not to relieve the pain of migraine headaches when they occur. Topiramate is in a class of medications called anticonvulsants. It works by decreasing abnormal excitement in the brain.
As far as transferring from one med to the other or even just onto a second one, my best advice is to contact your doctor and have them write up a chart. Otherwise, increase 50mg every 5-7 days with your Topomax and decrease 50mg of the Dilantin every 5-7 days until you are off the Dilantin entirely and on the prescribed amount of Topomax daily.
