Here in the Us a lot of insurances won't pay for procedures unless PT is tried, and failed, first.
Pain clinics here are quite good for bone pain - arthritis in the spine pinching nerves, sometimes steroid injections for bursitis and tendonitis. Some of the docs are quite good at addressing fibromyalgia also.
Meds for fibro is kind of try them all and see what works for you. They do seem to work for some people. For me, the so-called "fibro meds," Lyrica, gabapentin, Cymbalta and such, didn't help at all. Neither did the psych meds. All I got was side effects, no pain relief, at least none that lasted more than a couple of weeks, even though I built up doses as I was supposed to. Muscle relaxants were no better, except for Flexeril, which gave me a bad case of serotonin syndrome - when I took it the same day as my tramadol.
Tramadol has been the best pain med. There is no "cure" for fibro, so reducing pain is all I can hope for. At least it's generic (read cheap), and comes in instant release, which I take during the day, and extended release, which I take in the evening. Tramadol wakes me up, except the ER form, but at least that one lasts through the night. I just have to make sure to take it before the IR piill wears completely off, as the ER will keep pain down but will not lower it. This, of course, is how it's working for me at this time, not necessarily anyone else.
