I'm glad to hear that your brother is a bit more comfortable today. They won't keep him on the dilaudid long, probably a couple of days. Hopefully whatever is causing his discomfort will ease by then.
I wanted to comment on what Zif said, as I have some experience with it. Like Zif mentioned, not saying this is what is going on with your bro, but it is possible. When someone is coming off of an addiction, especially within the first few months of getting clean, there is a pretty big risk of entering another addiction. And pain meds are usually the culprit. Robert was an alcoholic, but had been sober for several years prior to his liver disease diagnosis. Afterwards, whenever he had a procedure or a hospitalization for a 'flare-up', he always seemed to 'require' pain meds when the average patient wouldn't need any - or more of it, or for a longer time period than average. He would scream and moan in pain - it was awful. I'm not saying he wasn't in pain, but he tended to exaggerate. Short of something else going on, there really isn't anything that would cause your brother such intense pain that it would make him scream and moan and need continuous IV dilaudid. There is zero pain coming from the liver itself because it has no pain receptors. Yes, ascites causes some pain and discomfort, as the fluid pushes against internal organs. And a swollen spleen can cause the same. But most pain can be controlled with low doses of a mild narcotic or higher doses of a non-narcotic. And the paracentesis would ease most of any pain caused by the ascites.
As I said, this is just something to keep in mind. There might very well be something else going on that could cause your brother such intense pain. And now that he is back in the hospital, maybe they can figure it out. He (and you) remain in my thoughts and prayers. I know how hard this is.
