Hi there, sounds as if you've really struggled in 7 years. I am the flip side, I had a partial colectomy due to cancer (90% of my sigmoid is gone, along with 15 inches of colon above that spot). I have multiple bm's per day. I struggle trying to keep my stool formed, but have never experienced obstructions or constipation in 13 years. I am of course reconnected (no bag) but since internally I have nowhere for stool to collect, I need to relieve myself all day long.
After reading about your hospitalizations, diet (mine is somewhat restricted but not to extent yours is), I arrived at one conclusion for you - an ileostomy. The ostomy board here is very active, I would post your questions over there and see if you receive positive feedback and success stories, I suspect you will. Sometimes it's not worth it to reconnect, if a person struggles as much as you have been and continue to struggle. I had a temporary ostomy for 2 months and it was not the end of the world. Far from it.
