dull pain under right rib for long time

Hi,
4 months ago I started to have a dull ache under my right rib.
I went to a gastroenterologist and they did a blood test and ultrasound.
From ultrasound they said i have an inflamed liver and from my blood results
the liver enzyme levels were as follows:

-Bilirubine direkt: 0.50 mg/dL (range: 0.2 - 1.3)
-Bilirubine totaal: 0.7 mg/dL
-SGOT: 97 U/L (range: 17-59)
-SGPT: 186 U/L (range: 21-72)
-Alkalische Fosfatase: 92 U/L (range: 38-126)
-Gamma-GT: 81 U/L (range: 15-73)
-LDH: 641 U/L (range: 313-618)

Doctor suggested me to loose
some weigt. After first test, I followed a diet and exercised regulary.
One month later, I did a second blood test with the following results:

-Bilirubine direkt: 0.43 mg/dL (range: 0.2 - 1.3)
-Bilirubine totaal: 1.1 mg/dL
-SGOT: 61 U/L (range:17-59)
-SGPT: 115 U/L (range: 21-72)
-Alkalische Fosfatase: 85 U/L (range: 38-126)
-Gamma-GT: 45 U/L (range: 15-73)
-LDH: 512 U/L (range: 313-618)

I continued diet and exercise became 68 kg from 74 kg. After two months from the second blood test, I had the following results:


-Bilirubine direkt: 0.24 mg/dL (range: 0.2 - 1.3)
-Bilirubine totaal: 0.7 mg/dL
-SGOT: 34 U/L (range: 17-59)
-SGPT: 53 U/L (range: 21-72)
-Alkalische Fosfatase: 81 U/L (range: 38-126)
-Gamma-GT: 28 U/L (range: 15-73)
-LDH: 480 U/L (range: 313-618)

As you see there is an improvement in my enzyme levels and the levels are in the normal range in the last test.
However. the dull ache pain below liver continues: it is very irregular: it comes and goes during the day.
Recently, I have developed some other symptoms: there is bloating, constipation and a lot of wind after meals.
Also, bowel movement and rumbling. When I go to toilet, I feel a relief. Also, sometimes the dull ache region gets itching a little bit and the dull ache region
is a little bit hot. No vomiting, no blood in stool and no appetite loss.
A few days ago, my gastroenterologist ordered a CT scan for the right hand side and it was normal: normal liver, pancreas, gall bladder, kidney and abdominal wall.
She said that she does not know the reason for dull ache pain below the liver.
What can be the reasons for the dull ache under liver and what should I do?

I'm glad that you've made such positive changes in your overall health! It is frustrating that the pain continues to bother you, though.

I had a similar experience. I'll tell you what my pain is about, but it doesn't mean that you are having the same problem.

I had upper right quadrant pain for a long time, too, and since I have a diagnosed bowel disease my gastroenterologist and I tried to figure out if the disease was causing the pain. My blood work was all normal. I had gallbladder studies done and they were normal. I had a CT scan of my bowel and it showed that my bowel was full of stool.

My GI and I concluded that the pain was essentially due to slow moving bowels. He recommended that I use miralax every day to keep my bowels moving and that did help. I ultimately switched from miralax to using a fiber supplement and I hardly ever have that pain anymore.

kazbern, hanks for your comments. how long did your pain last? was it a dull pain? did you get a relief from this pain when you expel wind or empty stool?

the pain I have/had in my right upper quadrant would be both constant and also occasionally spike up. i think that was due to colon spasms. I would only get relief by emptying my bowels, not by passing wind.

Sorry you're having these issues. I too have pain on my right side. With your liver enzymes being high, your gall bladder may be giving you issues bc its working harder to compensate for the liver. Have you had a ultrasound or did they check it for gall stones?

before u went into a diet and to lose weight was ur stool brown or yellow or other color. I know u mentioned that it wasn't bloody or dark.

9 months ago I had a sharp pain in my right quadrant.
after ultrasound and blood results, dr said I have faty liver,
my alt and ast enzymes were 2-3 times higher than normal range.
after following a diet, enzymes became normal.
however, then I started to have IBS kind syptoms with pain continuing.
i did additinally a ct scan, occult blood test and everything was normal.
then, after a time I almost became normal with no pain and with normal
bowel movements. I was normal for about 6 weeks...

then, i started to have an external homerroids but it was healed
after 6 weeks. during that time I just had a little gas problem.
i wanted to solve this gas problem also and then went to another doctor.
he did an upper endoscopy and said there is h pylori infection.
i started to use biaxin and two other antibiotics for the h pylori treatment.
one month after h.pylori treatment, i started to have diarrhea, burning in the anus.
after a while during one week, i observed blood in the stool when I had this burning.
blood was from rectal region since it was fresh.
then, burning became normal and no more blood in rectal area.
i think that blood was due to an anal fissure because it was giving pain during tiolet.
now, it is last 1 month that again I have IBS kind sypmtoms and pain mainly in the right quadrant.
sometimes (but rarely) also pain in the left quadrant. the pain is very irregular and sometimes
it is at a minimum level.
no fever, no weight loose and no blood in the stool.
i have excessive gas and small water from mouth when wake up.

What is my problem? What should I do?
