High ANA 1:640, Ena screen Negative, DNA antibody IFA screen negetive, doctor says No lupus

Hello,

I am in lot of confusion and anxiety. I dont understand what these results mean.

I have been complaining about pain in my neck behind the right ear , on my right cheek bone,

and tright temple that come and go. My dr thought it could be bacterial infection and as a precaution gav amoxiccilin fr 3-4 day course twice a day 300mg.

Then i complained to him about pain in upper back and muscle spasma that comes and goes.

recently in the last 2 weeks i have been experiencing pain in my knees{sensations/vibration}, buttocks, that radiates down my legs. I feel it more during sleep and hardly get 5-6 hrs sleep daily.

I feel hard to get up from bed in the mornings.

I got my blood work done and everything was normal, normal ESR, normal LST LFT, normal T4 and TSh ultrasensitive normal ( no thyroid).

But Anti nuclear Ab Qt was >= 1:640 homogeneous and speckled patterns, ANA screen with reflex positive.

ENA screen(sm, sm/RNP, ss-A(ro), ss-B(La and scl70) Negative

also DNA antibody IFA Screen Negative

My Dr says there is nothing to worry and its just inflammation and sciatica and prescribed me for physical therapy. I am currently taking gebapentin before goin to bed.

Im  26 M and of Asian origin. I am very confused and depressed as i fear it could be lupus.

My doctor refuses to diagnoise me for lyme or refer  me to a Rheumotologist.

Please help me : Do you think its just inflammation and sciatica

Prior medical history:

Mild gastritis, h. pylori bacteria and was treated. Aug - Nov 2010

I had Anxiety, panic attacks, nausea, depression  Aug- NOv 2010

I was asked to get tested for TB because my Gfriend got Active TB and i was looking after her for 2 weeks - as a precaution

My Result positive PPD - Feb 2011 but  Negetive chest xray - suggesting No active pulmorary disease

i was put on Isoniazid for two months- side effect facial numbness, tingling in fingers-

 but discontinued after second opinion and negative chest xray

Mri of brain - Normal - Mar 2011

I took TB Gold QFT test - reported Negative - No M. tuberculosis identified - May 2011

just to reconfirm i dont have TB

Please help me - i am having sleepless night with join pains and fear of auto immune disease.

Homogeneous pattern is most specific for SLE.
I have a positive speckled pattern and negative inflamatory marks, but my doctor told me I do have SLE. Also he's treating me with DMARDS.

Have you considered going to a different Rheumy? Some will treat the symptoms instead of totally basing the diagnosis on just the bloodwork.

i want to see a Rheumy, but my primary Dr doesn't want to refer me to a Rheumy.
I am on HMO and i have to go through my primary for any referral to a specialist.
Is there a way where i can see a Rheumy by myself without a referral?

Do you think there are people who have homogeneous pattern and have no lupus?

If I was you I'd change primary doctor. I was on HMO and my primary wouldn't do anything for my leg swelling but give me diaritics and wouldn't listen to my complaints. I switched doctors and the new doctor helped me a great deal.

Everything I've read on Homogeneous pattern said 97% of people with that pattern wind up with SLE.

Look at the Lupus.org site they have tons of information.

The pain could be a muscular and joint issue from Lupus. Your so young, if you do have Lupus, your antibodies to it probably won't show up for awhile, however, your ANA is high enough to be Lupus, but if your having normal blood work beyond the ANA, normal RBC's, WBC's, platelet counts, ESR, and other inflammatory markers, even if the disease is present, you probably aren't going to get it acknowledged. Sorry to say that, but I know that is how doctors are. They want to see raised inflammatory markers, even if Lupus can occur with a perfectly normal ESR. I have a homogenous pattern, now a nucleolar pattern also, and the antibody, and my ANA is as high as yours, I have symptoms to fit the criteria of Lupus, and severe fatigue, doesn't matter, because I have a normal ESR. I guess there are some doctors that won't judge whether you have the disorder or not mainly on what other blood work shows, but most is going to. They also look for joint swelling, rashes that occur with Lupus, and inflammation or fluid on your heart and lungs. When you first get the disorder, it might take some time for certain things to develop, and then again, that might not necessarily happen. I'd tell my doctor like it is, you either give me a referral or I walk. You might have little luck with a rheumatologist too but there is no harm in trying. I don't know what kind of medications your taking, but you might have some luck with a muscle relaxant at bedtime, try neurontin or lyrica for the sensations and pain associated with that.

Do people die with SLE if left untreated?

What are the worst complications of SLE?

I fear because Im 26 M and plan to get married this year. My Gf is worried and would like to now more about it.
i never heard of Lupus before and I dont know why it is showing symptoms all of a sudden.

can there be other diseases or people with no disease of lupus have ANA 1:640 and other lupus panels negetive but having joint pains, neck pain, hip pain, headaches, sinus, GI problems?

Can ANA 1:640 be false positive even with the above mentioned symptoms??

If SLE is left untreated it can cause further problems in other areas (heart, lungs, digestion, brain, etc.). These are sometimes fatal problems. There is no cure for lupus. It is only managed for your life.

Please go to Lupus.org and you will find a wealth of information about lupus.

Before Feburary of this year "Lupus" was just a word to me I heard once. It hit me suddenly one day and hasn't loosen up it's grip yet.

Please don't let the stories of Lupus scare you. Many of us do quite well for long periods when the Lupus is "quiet". Often it is when we quit being diligent about self-care and medical treatment when flares occur. (Although sometimes flares have no rhyme or reason whatsoever!)

That said, until/if you have a diagnosis of Lupus from a Rheumy you should not be reading up on it!! It will only scare you, and other Drs like to think they are able to diagnosis between various auto-immune disorders when that isn't the case. A positive ANA can mean nothing, or it can mean any of several auto-immune disorders.

If you do not feel like you are getting proper medical treatment and your symptoms are not being taken seriously, find another doctor. Surely there is another available, despite a HMO. Or spend money - health is more important than money.

After reading your posts several times, I wonder if you are a person prone to high levels of anxiety? Stress and anxiety can produce a number of the symptoms you've mentioned.

Go back to the 3 basic main things (1) Are you getting deep restorative sleep? Do you feel rested when you awake? (2) Are you eating enough/too much? Healthy foods at regular intervals? Do you drink enough water? (3) Do you get mild to medium exercise 3-4 times a week? That is, do you walk at least 20 minutes 4 times a week?

And after that -- are you taking things too seriously? Worrying about things you can't possible predict or control? Try to keep you thoughts to enjoying the moment rather than obsessing about the past or the future?

Those are the questions I ask myself when I start feeling symptoms. Often I've quit paying attention to some basic part of life and undermining my own health!

Hang in there, and if you want/need to see a Rheumy, call your HMO and see what you can work out!! It never hurts to ask!

PS. 1:640 is a medium ANA, not necessarily considered "high".
