1. Has your illness affected your sex drive?

2. Have you noticed any reduction in your semen?

3. do you have any discomfort, pain, numbness in your penis?

4. Has your GI tract been affected by Lyme/coinfection?

5. Has your bowel movements changed?

6. Do you have, or had long bouts of diarrehea, or soft mush stools?

7. Do you ever feel nautious, or worse health after a bowl movement?

8. Do you ever have cramping, sharp pain in stomach, or excessive gas?

9. have you ever had blood, or mucus in your stool?
