I haven't heard specifically of zith/bactrim, but I'm not in the medical field, I know the combo stuff only from research. Biaxin/bactrim is often used for Bart and Biaxin is very similar to Zith. Zith is also used quite often in a combo with Rifampin for Bartonella. Plus zith serves more than one purpose in being mild to moderately active against bart and babesia as well as being quite effective against the cell-wall deficient form of Lyme.
He may just want to see how you do first on this combo. I've usually done better herxing-wise when I added things in slowly one at a time. I recently made several changes at the same time and had a MASSIVE herx like I've never had before. It almost gave me a complete breakdown.
