I am not sure we should be filling this out, first of all the person doing the research , has no prior posts, how do we know it's not a insurance company or some scam. I am sorry, but I have already been pushed around enough by doctors, and people telling me what I have and what I don't, so until a moderator lets me know it's alright, I am not filling out anything.

that's what I noticed too: no real name, no emailaddress, no address, no information whatsoever. Not even on the survey itself...

Even the survey itself has writing mistakes in it.

BUT: it is indeed an anonymous survey :-)

No name, no explanation of what the survey is for, no contact info.

I would certainly like to help with giving info to causes that will help with education and understanding, but without any idea or way to verify what the info is used for, I will pass.

If this is sincere, please give us some way to verify who you are and what the info will be used for.

I'm so sorry that I've been away today - had to do some things with/for my Father-in-law.

Actually, this is not allowed on this site without prior approval from Administration on this site. I haven't received any notifications from Admin to allow this, so until I am told that this poster - Lyme_researcher, gets this approval, this will not be allowed.
