I think a lot of it is caused by natural die-off. If you take an herb to strengthen the immune system, but not nessessarily antibiotic in nature (ie astragalus or cat's claw) without taking some kind of anti-inflammatory/cytokine inhibitor (ie japanese knotweed) symptoms (especially fatigue and body pain) can ramp up pretty quickly - first hand experience here.
I don't think die-off is responsible for all of it though. I think spirochetes need to be physically chewing through cartilage to produce deterioration. Bartonella needs to have infected bone marrow to produce bone pain. The bacteria need to be present for the body to respond with cyclic fever responses.
