I have been tested for Ehrlichia, Rickettsia, and Bartonella locally here in BC. The results were "current infection unlikely or past infection where antibody titres are below detectable level". I am unsure if the Canadian co-infections tests are just as unreliable as they are for actual Lyme. I will check the ILADs & Lymedisease.org websites to see if they can direct me to an LLMD

It sounds like you may have been having a herxheimer reaction when you went back on antibiotics the second time, although I can't say for sure, since I'm not a doctor.
A herxheimer, although it feels terrible, is good in the sense that it means the antibiotics are working. Since the original antibiotics worked so well, my suggestion is that you go back on those, assuming your ND agrees.
Many people take Malarone and don't have vision problems, but you need to do a special kind of eye exam every six months; I think it's a retinal exam (?)
Malarone works better in conjunction with antibiotics.
Have you been tested for both Lyme and coinfections, and if so, what were the results?
I highly recommend that you see an LLMD; that's the only way you will get proper help. Either that, or at the very least, your ND needs to go by the ILADs guidelines.
Many people fly to go see their LLMDs. Are you sure there's no one in BC? I know that there are LLMDs in Washington state, certainly in Seattle; I just don't know their names. My guess is that there has got to be someone in Vancouver.
Go to the ILADs or Lymedisease.org websites, and they will give you instructions on how to find an LLMD.
