Nystatin is a "local" anti fungal for the GI as opposed to a systemic anti fungal, which is hard on the liver and shouldn't be taken long-term. When you take a systemic anti fungal you have to get your liver enzymes checked monthly or more frequently. I take both and also take supplements to help keep the enzymes under control (parsley, milk thistle seed and burdock root). After the first month on the systemic anti fungal my enzymes shot up but after I started taking the herbs, they went right back down.
Regarding the nystatin—it can be taken long-term with little to no side effects that I know of. I strongly recommend the powder form—it is more pure (the capsules have other stuff in them) you add the powder to a little water (tastes a little like clay) and it actually helps coat your mouth, esophagus and stomach. It's also much less expensive if you use a compounding pharmacy (near you or online). I can recommend a very good one in San Diego.
