I don't know if sayyadina started them all one by one - I'm assuming so, as they say that is the best, so you can more easily gauge which herbs are doing what for you, but seems to be on about all of Buhner's protocols for those infections, and a little extra.
Buhner has said that all of the herbs he recommends can be combined. Though, I think he cautions that one looks into whether or not the person themselves might have an issue.
As for sources..
For whole herbs (to be put info capsules, made into teas, tinctures), I feel most either go through one of these:
1stchineseherbs.com - they've got a lot of the major players there, as well as a bunch of other stuff.
Though, I've found that Mountain Rose (www.mountainroseherbs.com) has better prices for some things. It made me question the quality, but I've ordered some essential oils from them that seemed to be the exact same thing as brands charging twice as much offer.
I've gotten a few things from a Starwest Botanicals as well.

Also, Herb Pharm products, wherever you can get them the cheapest, for general support stuff. Like, before I had an issue with the alcohol in the tinctures, I found their red root to be quite nice.
Nutramedix is good. They're a bit more expensive, but they offer a couple things you can't find anywhere else that might be useful to you, especially their detox stuff. Pinella and burbur.
