Was at first skeptical of iSpot because it is a 'lab derived' lab test. Having been in lab testing for 15 years I know there are a lot of bad testing out there and Drs. and pt. are often not very savvy about testing.

This lab (Pharmazon Labs) opened up all its research and methods for scrutiny by the science community which is unusual for a lab derived test. They often hide behind not giving away patent secrets but this lab didn't do this.

Elisa and Western Blot tests are designed to give a 100% yes. With that sensitivity you lose the ability to pick up the people that 'may' have lyme disease.

After doing a few thousand tests I got more confident in the tests reliability.

It not a simple test. It doesn't directly measure antibody or whether a bacteria is present. It's a functional immune test which is a while different set of rules and is performed in a different manner.

Its a T cell based assessment to identify the presence of circulation borellia bergdorferi proteins.

We take your WBCs and concentrate the T cells and expose them to borellia proteins.

If any of your immune system have found a BB protein and taken it to your lymph tissue for processing. If 1 out of 1,000,000 cells is affected it picks that up so it's extremely sensitive and accurate. It tells activity level. You get a number associated with how many T cells are reacting to the BB proteins.

So the T cells are in a dish and they inject the BB proteins into the dish and if the person doesn't have lyme and has never been exposed the the cells don't react. If they react it's because they have been exposed to BB and the reaction causes a "spot' to form. Hence the name iSpotlyme.

You gauge how much protein activity is in the body by how many spots are formed in the dish.

>25 spots = High Activity of BB proteins

Due to the fact that T cells turnover (or are replaced) very rapidly, this test can also be used to monitor the pt. response to treatment because it gives you a snapshot of what is happening right now.

During acute lyme you see higher spots, >50 spots

Chronic will be lower as the immune system is becoming more depressed.

The test also measures the baseline activity of the T cells before the BB protein test.

<10 spots shows 'some' kind of infection or another co-infection.

Another test in the iSpot testing is to see how robust your T cells are by bombarding them with a T cell irritant that makes them go berserk. If they don't go berserk it may mean they are immunosuppressed and it also changes how you interpret the rest of the results. If their T cells are working at half of a normal person, a lower spot count may actually be a strong BB test.

The diagnosis of lyme disease is still clinical but this is not followed very often in medical setting because to get an ICD-10 diagnosis code you have to be positive with the Elisa and Western Blot which is appalling because the CDC doesn't proclaim that lyme can only be diagnosed with a lab test because they know lab testing is inherently imperfect.

The method used for the iSpot was borrowed and modified from a gold standard test for tuberculosis that has been used for the last 15 years.

Oh wow, that's not what I wanted to hear. Dang. How much did it cost?

When did you get it in your illness? Were you better already? It picks up the T cell response to circulating proteins so maybe did you not have any by then?

He talked a lot about the 3 different tests that the iSpot did... were all the results negative?
