I had the exact same thing happen to me last October. I looked down at my legs one day and collagen loss occurred in my knees, ankles, and my calf got smaller. And also in my right calf an actual indention.
It was weird. My left calf got smaller than my right but my right calf was the only one with the "indention". Strange.
Here's what worked for me. I started taking protein and collagen supplement daily and then I started working the muscle out.
I would do a variety of "heel raising" exercises similar to what professional dancers do. It's a very good workout on the calves. You may see those ballerinas do it but it's a work out. And then you can always increase your reps as you get stronger.
I did this every other day or every other 2 days for multiple months. I went very light with it and only started to work out my calves much harder recently (meaning in the last month).
Results:
Both calves grew. I would say 30-40% growth towards it's original size. With lyme, one side often response better than the other but both sides responded. The dent that felt like a crater in my right calf is being filled. It's hard to explain. It's no longer a big round indention, but there's one huge muscle fibers cutting across it almost filling it in but not quite completely. I suspect the more I stay on the physical therapy the more it will continue to fill in.
Basically what I'm trying to say is you can re-build muscle after lyme breaks it down. HOWEVER, I would also suspect one would have to get the bacterial load down to make gains.
