Have you read any of Buhner's books? They're the best resource for the herbal protocols that I've read so far, and aside from his three lyme and co books his book on herbal antibiotics is also a very useful read. Lots of ideas to work from and make tweaks to your protocol based on your own symptoms and instincts.
A big part of his approach is supporting the different organs and the immune system which makes a lot of sense to me - I'm working on being inhospitable ;-) I don't have his books with me right now but if I remember correctly the main antibiotics are cryptolepis and andrographis (babesia especially), houttuynia, isatis, alchornea, and sida (bartonella, although I think sida is used as much for it's protective qualities as anything). With the CSA in there too it sounds like you're on all of the antibiotics except Isatis (it needs to be used only for short periods though) although with the combinations it's harder to experiment with higher or lower doses of each which is something I'm exploring right now, especially for babesia.
FWIW, if I remember correctly some people use Red Root for tinnitus and Buhner has a special protocol for chronic fatigue which might be helpful (I haven't tried it yet myself).
I'm sorry to see from your signature that it sounds like you're working with all three (BBB), that's no fun at all. I'm hoping you find the right combination of herbals Serfr to make some progress. It seems it takes a lot of slow trust and some experimentation and I'm still very much in the middle of trying to understand it.
