I think the die-off is a good sign that those are effective treatments for you. The trick is finding the right pacing and not getting overly aggressive if die-off symptoms are never tapering off or unmanageable.
Finding a good llmd would be helpful, but it would have to be someone who fully understands that with your MCS you have to have a very careful, slow, methodical approach to the treatments. Some docs mean well but can get overaggressive too quickly with treatments. This is all just my personal opinion. I think some patients can handle a very aggressive protocol right off the bat, and others will just tank with that approach. Which is why I think a conservative approach initially is generally a safe hedge, can always increase the meds if patient is tolerating protocols adequately.
Given your symptom make-up and sensitivities I think the herbal route is good especially for now, it will help chip away at the pathogen load. Keep going slowly and methodically would be my advice. The Buhner books are a solid framework. I'd also research llmds in your area so that you know your options. Especially if you are having trouble figuring out next steps and treatment framework, having a good doctor with a lot of experience guide and provide treatments can be very helpful.
Are you doing a lot of detoxing? this can be very helpful to help increase ability to tolerate treatments and also manage symptoms.
Btw, given your symptoms and personal history it would make a lot of sense that bartonella is involved in your health issues. The provocation method (ie take antimicrobials, gauge response) is a great indirect confirmation of underlying chronic infections in my book.
