Metronidazole aka flagyl, was one of the first breakthrough antibiotics in lyme patient history, Dr. H reported of some lyme patients getting out of their wheel chairs with this antibiotic.
It's both antibacterial and antiparasitic, so without a doubt it probably hits babesia partially, Dr. Sapi reported it to works really on biofilms too.
So yes, you can herx on Metronidazole(flagyl) a lot. Even for me, Flagyl was one of the first breakthroughs I made in treatment, most antibiotics weren't working for me much, until I used flagyl, zithromax, and malarone.

I had heard about how Flagyl worked for some people with long-term Lyme, and so I got another doctor to prescribe the antibiotic Flagyl for me and I went on Flagyl for three months. And that was when things turned for me. I got significantly better on the Flagyl and then I thought, "Whoa, OK, maybe I can try to get pregnant myself. And I did. So I had Ben, and then a couple years later I did IVF and I had twins. I was still working my two days a week, and now I had three kids, and I still had a lot of pain problems but I was much better, because when I would have these Lyme flare-ups, I would just go on the Flagyl and it would help.
I've heard both Tindamax and Alinia properties are similar to Flagyl, may be worth a try as well.
My guess is, the one's that really respond the most with flagyl are the one's who are infected with Babesia, although it may be erradicating biofilms and spirochetes as well for some.

I've been on no lyme treatment for a year as I was in a very dark place just curled up into a ball and watched life go by.
Feeling absolutely horrendous exhausted fatigue chronic pain flu symptoms etc
More exhausted fatigue that traditional lyme patients lead me to believe babesia, also look for anxiety, night sweats, hard to pee, sweating, racing heart, raised liver enzymes... You may only have a few of these, others not so much at all.
