"Pain free A revolutionary method for stopping chronic pain" by Pete Egoscue
Its broken into body parts, arms, legs, back, hips, neck. Worth a checkout at the library. They are simple exercises to get your body into proper movement patterns and strengthen muscles to hold it there.
Sounds like a good follow up to Clair Daveis "Trigger point Therapy Workbook" and "Frozen Shoulder Workbook".
