My bottle says preferably on an empty stomach, but I've been taking mine with a meal. Would like to hear what others have to say about
this as perhaps empty stomach is more effective?

I remember reading that NAC can cause nausea, so even if it's absorbed better on an empty stomach, maybe take your first few doses with a bite or two of something, just in case? I have possibly an outsized fear of nausea. ;)

i take mine right before a meal also
my guess is it is easily absorbed and so taking with a meal should just spread the absorption out which is no bad thing.
supplement taken in this way can potentially suffer reduced absorption if eaten with a high fiber meal - but i cannot tolerate a great deal of fiber at present so i am not too concerned

My bottle says to take it with full stomach and it has been effective likewise. i take 600mg a day. Nac is a powerfull biofilm breaker and it crosses the blood brain barrier were bacteria can built biofilms, in my case i have brain biofilm because when i move my head i feel cracks like if there are empty spaces on the brain from biofilms already eliminated and debris inside that should take a sometime to dissapear and removed by the neutrophiles/macrophages. I am just certain about
this, there is not other explanation. This after using abx and nac for 2 years and a hald now.

great - I would say go steady, to begin with - it's known to be anti-microbial either directly or indirectly for some organisms - either by boosting glutathione or by its biofilm braking effect
this time i started at around 0.5g 2x a day and worked up from there
but i recall using it before where i jumped straight in at 2g 2x a day and had pretty strong herx like effects

My pain medicine dr just prescribed some, he said to take it regularly and give it time as it takes a while to build up, which I hadn’t realized

NAC is an amino acid and some amino acids compete for absorption. NAC, therefore, should be taken away from protein foods. I am not an expert, but all the literature has told me this. So I guess it's ok if you take it with food, just not with a steak or something like that.

Does it give anyone else severe heartburn? Almost every time I take it I get bad heartburn. I usually don't get heartburn.

No heartburn for me. I've been mixing the powder with a little water and drinking it 3x daily, sometimes on an empty stomach, sometimes not.
