Fatigue is typical with Doxy and the tetracylcine class of ABXs.
In my own personally experience, this fatigue subsided once I was off of it and then I felt amazing for months. Eventually, I relapsed and so other drug combos were needed. Doxy only targets the spirochete form and actually aggravates the persistent forms of Lyme and so it needs to be combined with other drugs and/tinctures for full spectrum healing.
LLMDs typically recommend an ABX and herbal combo that targets all 3 forms and 4
locations. My original (non neuro) treatment that worked very well was pulse dosing: Doxy+Tinidazole (tindamax) combined with Serrapeptase and tinctures. If your LLMD is recommending this route definitely jump on it. If your Lyme has turned neuro you will need to substitute your Doxy for Minocycline or something else which has better blood brain penetration.
Update: my Lyme is actually in remission now so disregard my old signature below
